This presentation will look at the medical record as it relates to patient care, payers and policy. It will focus on requirements that have been in place for many years and continue to present challenges for HIM leaders and staff; as well as newer requirements for documentation. Solutions to challenges and best practice examples will be provided as well as a list of required policies. Joint Commission and CMS will serve as the basis for the presentation.
