NHC of Anderson, a skilled long term care facility, currently has an opening for a Unit Record Coordinator in the HIM Department with the potential for advancement.

Job Description:

The Unit Record Coordinator will be responsible for coding admission diagnoses and updating/resolving diagnoses as needed, Release of Information, reviewing medical records for completeness and accuracy during stay and at discharge, assist with Quality Assurance Program as needed, and assisting the Director of HIM as needed.

Required Qualifications:

Successful candidate should be a graduate or soon to be graduate of an Accredited Health Information Management program.

RHIT

Introduction:

Full time CDI Specialist position immediately available in 286 bed acute care facility in Orangeburg, SC.

Job Description:

Reviews the integrity of documentation of care provided and coded and facilitates concurrent modifications to clinical documentation to ensure commensurate reimbursement of clinical severity and services rendered to patients. Performs concurrent record reviews on selected admissions, reviews core measure documentation, and provides feedback when appropriate. Facilitates modification to clinical documentation to accurately reflect patient severity of illness and risk of mortality through extensive interaction with clinical, HIM coding staff, and other caregivers. Audits records of denials identified through various sources such as MAC, RAC, MIC, CERT, and commercial payors utilizing criteria and regulatory guidelines.

Required Qualifications:

Registered Nurse with a current SC or compact state license or a RHIT, or RHIA with 2 years of medical audit, utilization review, coding, or case management experience and knowledge of CMS guidelines, The Joint Commission standards, ORYX core measures, and Interqual/Milliman criteria.

Introduction:

Full time coding position immediately available in 286 bed acute care facility in Orangeburg, SC.

Job Description:

Performs ICD-10-CM and CPT 4 coding including evaluation and management codes for emergency department physicians and facility levels. Abstracts records as assigned. Monitors weekly report of un-coded records. Performs ongoing record review and quality assurance functions to validate coding documentation is complete, legible, timely, and meets regulatory requirements. Mentors Coder I staff.

Required Qualifications:

High School Diploma or GED and one (1) year secondary training and one (1) years experience in medical coding and have credential of RHIA, RHIT, CCS, CPC, CPC-H, RT(R) or RN.
