Job Description:

Essential Functions and Responsibilities

  • Create and Produce coding training and education content for internal and external clients
  • Deliver training and education in a classroom setting or in a web-based environment
  • Provide assistance in all aspects of LexiCode’s Coder Development Program (CDP) including ongoing curriculum and materials assessment, development and updates for all record types

Required Qualifications:

  • RHIA, RHIT, or CCS credential from AHIMA
  • 5 or more years of coding experience in an acute care hospital setting
  • Demonstrated proficiency coding inpatient and outpatient record types
  • Knowledge of various reimbursement models

Job Description:

This position is responsible for performing concurrent review of the clinical documentation in the medical record. These duties include but are not limited to:
• Using clinical judgment and clinical analytical skills, queries the medical staff and other care givers through written and verbal communication to obtain accurate and complete documentation that supports the severity of patient illness and risk of mortality, as well as allowing accurate assignment of ICD-9-CM codes that reflect the highest level of specificity that supports treatment provided. This includes querying for physician intent to assure identification and documentation of all co-morbidities and complications.
• Demonstrates an understanding of the importance of and makes an effort to capture all potential secondary diagnoses for profiling purposes.
• Uses clinical decision-making to identify “hints and clues” in the chart that would guide the physicians documentation toward clearly defined definitive diagnoses rather than non-specific signs and symptoms.
• Demonstrates creativity and enthusiasm while pursuing departmental and organizational goals.
• Maintains the ability to be flexible and prioritizes daily responsibilities.

Required Qualifications:

  • Minimum of five years clinical experience in an acute care setting, preferably in medical/surgical with a current SC RN license.
  • Excellent communication and critical thinking skills. Computer literacy required.
  • Ability to work independently and meet deadlines and schedules is required.
  • Knowledge of care delivery documentation systems and related medical record documents.
  • Knowledge of age-specific needs and the elements of disease and related procedures. Strong broad-based clinical knowledge and understanding of patho-physiology and pharmokinetics.
  • Able to conduct education sessions and in-services to physicians and ancillary staff and able to write articles for medical staff newsletters on disease processes, how to document, and implications for aggregated public data.

Full time CDI Specialist position immediately available in 286 bed acute care facility in Orangeburg, SC.

Job Description:

Reviews the integrity of documentation of care provided and coded and facilitates concurrent modifications to clinical documentation to ensure commensurate reimbursement of clinical severity and services rendered to patients. Performs concurrent record reviews on selected admissions, reviews core measure documentation, and provides feedback when appropriate. Facilitates modification to clinical documentation to accurately reflect patient severity of illness and risk of mortality through extensive interaction with clinical, HIM coding staff, and other caregivers. Audits records of denials identified through various sources such as MAC, RAC, MIC, CERT, and commercial payors utilizing criteria and regulatory guidelines.

Required Qualifications:

Registered Nurse with a current SC or compact state license or a RHIT, or RHIA with 2 years of medical audit, utilization review, coding, or case management experience and knowledge of CMS guidelines, The Joint Commission standards, ORYX core measures, and Interqual/Milliman criteria.

Performs ICD-10-CM and CPT 4 coding including evaluation and management codes for emergency department physicians and facility levels. Abstracts records as assigned. Monitors weekly report of un-coded records. Performs ongoing record review and quality assurance functions to validate coding documentation is complete, legible, timely, and meets regulatory requirements. Mentors Coder I staff.

Required Qualifications:

High School Diploma or GED and one (1) year secondary training and one (1) years experience in medical coding and have credential of RHIA, RHIT, CCS, CPC, CPC-H, RT(R) or RN.
